Program Guidelines
- Patient's portion of the bill is due the day of service.
- Membership must be paid in full at the time of enrollment.
- Membership is non-refundable and non-transferable.
- Cannot be used in conjuction with another dental plan.
- Plan discounts do not apply to Lab fees and Products (e.g. toothpastes, rinses, electric brushes, etc.)
- Services must be used within the 12-month period. Unused services do not roll over.
- Membership will auto-expire unless renewed by the patient.
